This is FlyGuy, a simple little game where you play as a fly who must dodge falling shapes while trying to fly as fast as his little fly wings can take him.
We've already released the Kongregate version, and are in the process of making a complete iOS/Android overhaul with several additional modes in addition to the "simple" Kongregate version (if there is enough interest)
